Healthcare and health insurance in Thailand: costs, hospitals, and the Koh Tao to Samui emergency route
How the Thai system works for foreigners: public, private and no state cover
Foreigners sit outside Thailand's public health scheme, so every visit is paid at the counter or by an insurer.
Foreigners are not covered by Thailand's public healthcare scheme, and without insurance payment is required upfront, in public and private hospitals alike (The Thaiger, digital nomad Thailand healthcare and insurance costs). There is no European style health card here and no reciprocal arrangement to fall back on.
Two parallel systems. Government hospitals are cheap and competent, with a basic consultation from 30 to 200 THB, long waits and limited English. Private clinics and hospitals are fast and English speaking, with a clinic consultation at 500 to 2,000 THB (The Thaiger, above). The gap between public and private is money and comfort rather than skill.
No visa makes you buy cover, with two exceptions. The Royal Thai Embassy in London lists the DTV documents as passport, photo, proof of residence, funds, a police certificate and proof of remote work, and health insurance is not among them (Royal Thai Embassy London, DTV). The LTR requires health insurance of at least USD 50,000, Thai social security or USD 100,000 kept in a bank account (Thailand Board of Investment, LTR visa portal). The Non-Immigrant O-A retirement visa requires insurance with a sum of no less than USD 100,000 or 3,000,000 THB (Royal Thai Embassy London, retirement visa).
So the decision is yours, and it is a risk decision rather than a legal one. Our view is simple: the ceiling matters more than the premium, because the bills that ruin people are the surgery and the evacuation, not the consultation. The next section prices both.

What a hospital bill actually looks like without insurance
A clinic visit costs the price of a nice dinner, while private surgery runs 100,000 to 500,000 THB or more and an evacuation up to 1,500,000 THB.
At a private hospital in Thailand a doctor visit runs 1,000 to 3,000 THB, an emergency room visit 3,000 to 15,000 THB, a night in hospital 10,000 to 30,000 THB or more, surgery 100,000 to 500,000 THB or more, and a medical evacuation 300,000 to 1,500,000 THB, with one writer quoted 230,000 THB for surgery after a motorcycle accident (Thailand Starter Kit, health insurance for digital nomads, 25 August 2026). The first two numbers are why people say Thai healthcare is cheap. The last three are why people buy insurance.
The deposit comes before the treatment. Private hospitals typically ask for 50,000 to 200,000 THB before a planned procedure and up to 800,000 THB before major surgery, unless your insurer sends a guarantee of payment (The Thaiger, expat healthcare in Thailand 2026).
On the islands the everyday numbers are smaller. A private clinic visit on Koh Tao costs 700 to 1,500 THB and a visit to Koh Tao Hospital 200 to 800 THB (Baanlyy, healthcare and hospitals in Koh Tao 2026). A cut that needs stitches, an ear infection after a week of diving, a course of antibiotics: all of that is an afternoon and less money than a fun dive.
The cliff is sharp and specific. It starts where a case stops being treatable on the island, because then the cost is treatment plus transport plus a private hospital on Koh Samui. A speedboat transfer to Samui runs 2,000 to 8,000 THB before any medical charge, and a hyperbaric session for a dive injury 15,000 to 40,000 THB or more (Baanlyy, above). Add a transfer, a chamber session and two nights in a private room and the bill passes 100,000 THB without anything exotic happening.
What that means in practice. Carry a card that can absorb a deposit, keep your insurer's 24 hour number in your phone rather than in an email, and know whether your policy pays the hospital directly or reimburses you later. On a reimbursement policy, you are the one producing the cash at the counter.

Motorbike accidents, licences and the exclusion that voids the claim
Riding without a licence valid for the bike can invalidate travel insurance, and a car licence does not cover a scooter.
This is the section that costs people the most money, so here is the sentence that matters: the UK Foreign Office warns that riding a motorcycle in Thailand without a valid licence "could invalidate your travel insurance", British riders need the 1968 International Driving Permit alongside their home licence, and helmets are required by law (UK FCDO, Thailand safety and security). A car licence alone does not cover a scooter, and a Koh Tao rental guide updated in July 2026 warns that without a valid motorcycle licence virtually every travel policy pays nothing, for the bike, the other party or your hospital bill (Wonderland Koh Tao, scooter rental). Rental shops rarely check. Your insurer will.
The exceptions are narrow and written into the wording. Insurers set conditions on engine size, licence class and helmets, and two policies popular with nomads draw those lines in different places. Why this matters more than any traffic fine. The biggest safety risk on Koh Tao is traffic accidents on rented scooters, on steep hills, sandy corners and uneven roads, while serious violent crime involving tourists is rare (Traveltomtom, is Koh Tao safe). Nomads.com rates overall safety on the island as bad (Nomads.com, cost of living in Ko Tao), which reads as alarming until you set it next to the road risk.
Add up a bad afternoon. A clinic visit is 700 to 1,500 THB. A fracture, a boat to Samui and a private hospital is a different order of magnitude, and one motorcycle accident surgery in Thailand has been quoted at 230,000 THB (Thailand Starter Kit). Then there is the shop's repair bill for the bike, with your deposit already in their drawer.
We are not going to invent your policy conditions. Wordings differ on licences, helmets, engine size and passengers, so read yours, and if it is ambiguous, email the insurer and keep the reply.
What to do before you rent. Get the permit at home with the motorcycle category, because it only repeats what your home licence carries and cannot be issued from a beach. Photograph the bike from every angle before you ride away. Wear the helmet on every ride, including the short one to dinner.

Diving cover, the recompression chamber and what a session costs
Koh Tao has had its own recompression chamber since 2023, open for emergencies 24 hours a day, and a session costs 15,000 to 40,000 THB or more.
If you dive on Koh Tao, the chamber is on the island: the SSS Chamber Network runs a multiplace chamber there with 24/7 emergency recompression and a dive emergency line on +66 81 081 9777 (SSS Chamber Network, Koh Tao), and Baanlyy dates the island's own chamber to 2023 (Baanlyy, healthcare and hospitals in Koh Tao 2026). Plenty of older content still sends divers to Koh Samui for recompression. With decompression illness, time to treatment is the whole game.
What it costs. A session runs 15,000 to 40,000 THB or more, and Baanlyy calls dive specific cover such as DAN close to essential for anyone diving regularly (Baanlyy, above). We have not verified 2026 DAN premiums or the prices of the short term dive policies sold in the shops here, so this guide does not print figures for them: ask two dive schools and your own insurer in the same afternoon and you will have current numbers.
What to check in your own policy. Whether diving is covered at all, to what depth, whether it requires certification, whether a training course counts, and whether chamber treatment and evacuation are both included. If you are starting a Divemaster internship, ask the school what its insurance covers, because the school's policy and your policy are different documents and only one of them is yours.
The other rule divers break. Leave 24 hours between your last dive and any flight (Koh Tao Tourism Association, safety). It applies to a medical flight as much as to a holiday one, which is one more reason the decision to move a patient belongs to a doctor.
And a practical one. Do not dive with an ear or sinus infection, however much the boat is already paid for. The clinic visit that clears it costs a fraction of the one that treats what comes after.

Healthcare on Koh Tao: the hospital, the clinics, the numbers to save
Koh Tao has a government hospital in Mae Haad with a 24 hour emergency service, private clinics open late or around the clock, and no full hospital.
Koh Tao has a government hospital rather than a full hospital: it sits in Mae Haad on 077 456 490, sees outpatients daily from 08:00 to 16:00 with a 24 hour emergency service, covers lab work, X-ray, dental care, deliveries and diving check-ups, and transfers serious cases to Koh Samui or the mainland at Chumphon or Surat Thani (Koh Tao Complete Guide, hospital, offices and services). For ordinary problems it is quick, cheap and adequate.
Private clinics cover the evening. Ocean Medical Clinic sits in Mae Haad opposite Krungsri Bank, opens 09:00 to 24:00 daily on 077 423 801 and lists dive medicine, a hyperbaric chamber, X-ray and blood tests; Thai Inter Medical Clinic is open 24 hours on 077 332 511; and Koh Tao Physician on Sairee Beach is reported to work around the clock (Baanlyy, healthcare and hospitals in Koh Tao 2026). Between the hospital and the clinics, an ordinary problem gets treated on the island the same day.
Save these numbers before you need them. In a medical emergency call 1669, Thailand's national emergency medical line. Rescue Koh Tao, the 24 hour land and sea rescue service in Chalok Baan Kao, also answers directly on 087 979 0191 or 1333 (The Funky Turtle, Koh Tao rescue; Koh Tao Complete Guide, above), the tourist police answer 1155 around the clock, and the local police are on 077 456 631 (Koh Tao Complete Guide, above). What the island cannot do. Anything needing a surgeon, an intensive care bed or a specialist. That is not a criticism of a small island hospital; it is the reason the next section exists, and it is worth understanding before you choose a policy or a place to live, particularly if you are coming with children or an existing condition.
One habit worth copying. Treat small wounds seriously and early. In a hot, humid, salty place a scraped knee from a scooter becomes an infected one faster than you expect, and the clinic visit that prevents that costs a fraction of the one that treats it.

Emergency to Samui: the boat, the cost and Bangkok Hospital Samui
Serious cases leave the island by boat, reaching Koh Samui in one to two hours for 2,000 to 8,000 THB before medical charges.
When a case is beyond the island, the speedboat transfer to Koh Samui takes one to two hours depending on conditions and costs 2,000 to 8,000 THB, with Bangkok Hospital Samui, Thai International at Bandon and Samui International as the nearest major hospitals (Baanlyy, healthcare and hospitals in Koh Tao 2026). The decision to move a patient is a medical one, made by the doctor on duty.
Where you land. Bangkok Hospital Samui is the usual destination for anything serious. The hospital says it was founded in 2004, has 50 beds, serves more than 60 percent foreign patients and runs a Hydrolance boat ambulance for areas including Koh Tao and Koh Phangan (Bangkok Hospital Samui, our story). It sits at 57 Moo 3, Thaweerat Phakdee Road, Bophut, runs a 24 hour emergency room on +66 77 429 500, and bills directly with Allianz, Cigna, AIA, April, Pacific Cross, BUPA, Aetna and William Russell, according to ThailandKnowledge (ThailandKnowledge, Bangkok Hospital Samui).
Direct billing is the difference between calm and chaos. With an insurer the hospital already deals with, a guarantee of payment replaces the deposit and treatment starts. Without it, you are producing a deposit at the desk. Check your insurer against that list before you need to, and if yours is not on it, ask how their guarantee of payment works at a Thai private hospital.
What to do in the first ten minutes. Get the patient to the hospital or the chamber, calling 1669, or Rescue Koh Tao directly on 087 979 0191 if you need help moving them, and only then start on insurance. Once somebody is being treated, call your insurer's 24 hour line, give them the hospital name and ask for a guarantee of payment. Photograph everything, including the bike, the scene and every piece of paper handed to you, because claims are made of documents.
And the number nobody wants to think about. A full medical evacuation, meaning an air ambulance or a repatriation flight home, runs 300,000 to 1,500,000 THB (Thailand Starter Kit). Pharmacies, dentists and routine care on a small island
Everyday care happens on the island in an afternoon, and anything specialised is a planned trip to Samui or the mainland.
Routine care on Koh Tao is quick and cheap, and specialised care is a trip you plan. The government hospital in Mae Haad covers lab work, X-ray, dental care, deliveries and diving check-ups alongside its daily outpatient clinics at 200 to 800 THB a visit, and the private clinics handle the same everyday problems in the evening at 700 to 1,500 THB (Koh Tao Complete Guide; Baanlyy). A dive medical, a tetanus booster, a rash that will not go away: island problems, solved on the island.
What we could not source, and will not invent. We found no published directory of pharmacies and dentists on Koh Tao that we would stand behind, so this guide does not print names and opening hours that might be a year out of date. Ask at the hospital or at Ocean Medical Clinic, both in Mae Haad, for a current answer.
Plan prescriptions around the boat. If you take something regularly, bring enough for the whole stay plus a buffer, carry the prescription or a photograph of the box, and expect to restock on Samui or the mainland rather than here. Chronic conditions and long term follow up are a mainland conversation, and they are also the part of your health that a nomad travel policy is least likely to cover.
Dive medicals are the one thing the island does more than anywhere. The hospital and the dive clinics sign them off, which matters if a school asks for a medical before a course, and it is worth doing early in the week rather than on the morning the boat leaves.
Before any flight after diving, the 24 hour rule applies (Koh Tao Tourism Association, safety), which is worth building into travel plans rather than discovering at the pier.
