Is public healthcare free in Portugal for foreigners?

The Servico Nacional de Saude is funded from general taxation and open to legal residents, not to citizens only. Your nationality does not decide the price. Your residence status and your registration at a centro de saude do.

What the abolition of user fees actually changed

Until 2022 an SNS appointment carried a small charge, the taxa moderadora. Those fees were removed from nearly every service by Decreto-Lei 37/2022 of 27 May 2022, and the rules in force in 2026 sit in Portaria 480-A/2025/1 of 30 December 2025: the only service still billed is a hospital urgencia visit made without a prior SNS referral, and the fee is waived when SNS 24 or a health centre referred you, or when the visit ends in admission. In practice a consultation with a family doctor, a referral to a specialist, blood tests, imaging and a hospital admission are not billed to you.

What you still pay

1. Prescriptions. The state subsidises medicines by category rather than covering them, so you pay a share at the pharmacy counter. Give your numero de utente to get the subsidised price.
2. Unreferred emergency visits. Turning up at a hospital emergency department without passing through SNS 24 on 808 24 24 24 is the one situation still billed. One phone call before you go is what keeps it free.
3. Anything you choose to do privately. A self pay specialist consultation at CUF Tejo in Lisbon ran 104 to 125 euros on the February 2026 price list, and adult urgent attendance 135 euros, before tests or medication.

Households whose average monthly income, divided by the number of tax filers, is at or below 805.70 euros in 2026 (1.5 times the IAS of 537.13 euros) are exempt from what remains, and pregnant women are exempt until the postpartum review.

Where the real cost hides

The SNS is inexpensive and slow in the places where demand is highest. The cost of public healthcare for a new arrival is rarely money, it is the wait for an assigned family doctor and the wait for a non urgent specialist appointment. That is the gap private insurance is bought to fill, and it is a separate decision from whether the SNS will treat you.
