Do I need private health insurance in Portugal if I have SNS access?
The SNS is real cover, not a token. User fees (taxas moderadoras) were removed from nearly all services on 1 June 2022, with the 2026 rules set by Portaria n. 480-A/2025/1 of 30 December 2025. Emergencies, surgery, maternity and chronic care are free at the point of use for registered residents. Walking into a hospital emergency department without an SNS 24 referral is one of the few things still billed.
Why people buy anyway
Access, not quality. At the end of June 2026 the SNS transparency portal counted 1,672,130 people without an assigned family doctor, concentrated in Lisbon, the Algarve and parts of the Setubal peninsula. Without a family doctor you have no easy route to a referral, so routine problems drift. Private insurance buys you a GP appointment this week, a dermatologist next month and a scan without a queue.
The visa requirement is separate
D7 and D8 applicants must show valid travel or private health insurance covering necessary medical expenses, including urgent care and repatriation, for the whole stay. The ministry document list sets no fixed euro minimum for this cover, so ask the consulate what limit it wants printed on the certificate. Being registered with the SNS does not satisfy this requirement, and consulates do not accept it as a substitute.
What it costs and what to watch
Basic Portuguese plans sit at 30 to 60 euros a month per adult, comprehensive ones at 80 to 150, with couples often quoted 80 to 140 combined. International plans that follow you across borders start around 85 euros a month per person. Two things reliably surprise people: permanent exclusion of some pre-existing conditions, and an age cap on new applications between 65 and 70 with most Portuguese insurers. Ask about the waiting period before non emergency treatment is covered too, since each insurer sets its own length.
A workable middle path
Register with the SNS for emergencies, chronic care and your children's vaccines, and hold a mid tier Portuguese policy for consultations and diagnostics. If you pay out of pocket instead, budget on the CUF Tejo February 2026 list: about 104 to 125 euros for a specialist consultation and 135 euros for an adult urgent attendance.
