How much does private health insurance cost in Portugal for a family?

Portuguese insurers price per person and per age band, then apply the household discount, so a family quote is built from individual premiums rather than read off a family tariff.

The 2026 numbers we could verify

The March 2026 rate cards put Multicare entry plans from 30 euros, Fidelidade from 35 and Medis from 40. Those are starting prices for a healthy adult in the standard age bands, not what a 55 year old with a history pays.

What we will not invent

None of the sources we checked publishes a per child premium or a worked family of four quote, so we are not going to produce one. Children are usually the cheapest people on a policy, but the only way to get a real number is to run a quote with your ages and postcode. Treat any site that gives you a tidy family figure without asking for ages as marketing.

The clauses that decide the price

1. Waiting periods. Non emergency conditions are typically locked out for 3 to 12 months, and maternity for about 10 months, so a plan bought this month does not help this year.
2. Pre existing conditions. Covered unless the contract excludes them, and exclusions are common. Read the exclusions list before the price list.
3. Age. Renewal increases of 8 to 15 percent a year are common after 60, and several insurers price older applicants out with tighter conditions or exclusions rather than a published cutoff, so ask about the policy's age ceiling before you sign.
4. Geography. A local plan stops at the Portuguese border. An international plan travels with you and satisfies a D7 or D8 file, which is why many new arrivals hold one for the first year and switch later.
