No family doctor in Portugal? How to use the SNS anyway

Having no family doctor in Portugal does not lock you out of the SNS: you still register at your centro de saúde, book same day consulta aberta slots, call SNS 24 on 808 24 24 24 for triage, and keep hospital emergency visits free by getting referred first. What you lose is a named GP who follows your file and pushes your specialist referrals. This guide explains how each door works, what still costs money in 2026, and where private insurance fits.

The shortage is not a foreigner problem. According to the SNS transparency portal figures reported by Observador, 1,672,130 registered users had no assigned family doctor at the end of June 2026. The Lisboa e Vale do Tejo region held about 1.18 million of them, or 70.5 percent. Newcomers tend to move to exactly those places.

How do you register at the centro de saúde without a family doctor?

You register the same way as everyone else, and the lack of a doctor does not block registration. The UK government's guidance for residents in Portugal says to go to your local centro de saúde with your valid passport and residence permit, and states that you do not need a social security number to register. You receive a número de utente, the number every SNS service asks for.

Two details save time:

1. The gov.pt service page (updated 12 May 2026) says foreigners receive the número de utente on their first visit to a public health unit, such as a health centre or hospital. It also warns that holding the number does not by itself guarantee SNS coverage of your care, so complete the full registration at your centro de saúde.
2. Portugalist reports that since July 2024 you can also register at a local Espaço Cidadão, which helps if your health centre is overloaded.

When the desk says no doctor is available, ask to be placed on the waiting list for a médico de família and note the date. Then ask how to book consulta aberta. One sentence covers it: "Não tenho médico de família, posso marcar consulta aberta?"

Where do you go when you need care without an assigned doctor?

You pick the door by urgency, and each one is free when used in the right order. Migaku's 2026 SNS guide describes the options for unassigned patients as SNS 24, walk-in consultations at the centro de saúde (consulta aberta or consulta do dia) and emergency services.

Why call SNS 24 before going to an urgência?

Calling SNS 24 first is what keeps a hospital emergency visit free. The health regulator ERS lists hospital urgência without prior SNS referral as the main service that still charges a taxa moderadora. The fee is waived when you were referred by SNS 24 (808 24 24 24) or a health centre, or when the urgência visit ends in a hospital admission.

User fees were removed from nearly all other SNS services by Decreto-Lei 37/2022 of 27 May 2022. The rules in force in 2026 also cite Portaria 480-A/2025/1 of 30 December 2025, according to the ERS page.

SNS 24 is nurse triage, not a switchboard. The outcome is advice to stay home, a booked appointment, or a referral to an urgência that carries the fee waiver with it.

Who is exempt from taxas moderadoras in 2026?

Households on low incomes are exempt from the remaining fees. ERS sets the 2026 economic insufficiency limit at 805.70 EUR of average monthly household income, divided by the number of tax filers in the household, which is 1.5 times the IAS of 537.13 EUR. You apply through the Área do Cidadão on the SNS portal, at a health centre or at an Espaço Cidadão. A D8 applicant, who must show at least 3,680 EUR a month gross in 2026, will sit far above that line, and the exemption matters less now that so few services charge.

What is the SNS slow at without a family doctor?

Elective specialist care is the real gap. The family doctor is usually the one who refers you to a hospital specialist and chases the referral. Without one, you rely on whichever doctor sees you in consulta aberta, and your file has no single owner. Acute care through SNS 24 and the urgência works; planned care for a chronic condition is where waits grow. If you need regular specialist follow up, plan for that before you move.

Is private health insurance in Portugal worth it alongside the SNS?

A basic local plan is often enough for speed, with the SNS doing the heavy lifting. Feather's guide, dated 28 April 2026, puts basic Portuguese plans at 30 to 60 EUR a month, comprehensive plans at 80 to 150 EUR, and international cover from 85 EUR. It quotes published starting rates of 30 EUR for Multicare, 40 EUR for Médis and 35 EUR for Fidelidade, and notes that most local insurers stop accepting new applicants between ages 65 and 70.

The comparison that matters is against self-pay prices. Live in Portugal cites CUF Tejo's February 2026 self-pay list at about 104 to 125 EUR for a specialist consultation. A plan at 30 to 60 EUR a month costs 360 to 720 EUR a year, so it pays off if you expect several private consultations or scans a year.

Read the exclusions before the premium. A June 2026 guide from Apoio Jurídico Imigração reports waiting periods of three to twelve months for non-emergency conditions, permanent exclusion of pre-existing conditions on some policies, and yearly increases of 8 to 15 percent as common for customers over 60. If you plan a pregnancy, check the maternity waiting period in particular.

Does SNS registration replace the insurance your visa needs?

No, the visa insurance requirement is a separate document. The Portuguese consular documentation list for residency visas asks for valid travel insurance covering medical expenses, including urgent medical assistance and possible repatriation, and does not mention the SNS as an alternative. Keep the certificate you bought for the visa separate in your head from the cover you actually use once you are resident. Checklist for your first months without a médico de família

1. Register at the centro de saúde for your address as soon as you hold a residence document, and leave with your número de utente.
2. Ask to join the family doctor waiting list and write down the date. Ask again every few months.
3. Save 808 24 24 24 as "SNS 24" and call it before any urgência visit that is not life threatening.
4. Learn the consulta aberta sentence and the opening hours of your centro de saúde.
5. Keep a basic private plan for speed and imaging if the numbers work for you, not as a replacement for the SNS.
